What this website collects, what we do with it and what you can ask of us. It describes what the site actually does, not a template.

Who we are
This website belongs to Clínica General La Gran Familia, whose office is at 3035 S Shiloh Rd, Suite 145, Garland, TX 75041.

This is not the HIPAA Notice of Privacy Practices
They are two different documents and it matters not to mix them up. This one explains what this website does. The Notice of Privacy Practices, required by HIPAA, explains how your medical record is handled once you are a patient: the clinic hands it to you in person and you can ask for a copy at any time.
Put simply: what you type into the form on this site is governed by this page; what happens in the exam room is governed by that one.
What we collect, and only when you type it
The site asks nothing of you to browse it. There is a single form, the appointment request, and it asks for five things: your name, your phone number, your email address, and the day and time you would prefer. There is a sixth field, the reason for your visit, which is optional.
That reason field can contain information about your health, which is why we ask you to use it sparingly: a general phrase is enough — "check-up", "sore throat", "DOT physical" — for us to know how much time to set aside. You do not need to write your history there, and you are better off not doing so: we will go through it in the exam room, where it is protected as it should be.
As with any website, our provider also keeps technical server logs (IP address, browser, time of access) for a short period and for security reasons. We do not cross-reference them with anything and we do not use them to profile you.
What happens to your appointment request
It becomes an email and is sent to the clinic's mailbox through Resend, the service we use to deliver mail. You receive a separate acknowledgement at the address you entered.
This website has no database. Your request is not stored on any server of ours: it exists as an email in the clinic's mailbox and in yours, and nowhere else.
We use it only to call you and confirm availability. We do not sell it, we do not pass it on and we do not use it for advertising.
The mailbox is read during clinic hours. A request that arrives on a Saturday night is not seen until Sunday, so if it is urgent, call us. And if it is an emergency, dial 911.
Email is not a secure channel
Neither ours nor yours. An email can sit on several servers along the way and can be intercepted. That is why we ask you not to email us detailed clinical information, lab results, policy numbers or identity documents.
For any of that, use the phone or the clinic itself. If you need to get a document to us, tell us and we will explain how.
Who we share data with
Nobody, as a commercial decision. Only the providers needed to make the website work are involved, and each one sees the minimum:
Cloudflare hosts the site and relays every request, so it sees your IP address.
Resend delivers the form emails, so it processes whatever you wrote in them.
Microsoft Clarity and Google Analytics measure how the page is used and set cookies of their own. Clarity also records the browsing session. You can switch both off from the "Cookie preferences" link in the footer, and we cover them in the cookie policy.
Google Fonts serves the typefaces; it receives your IP address when they download, but sets no cookies.
The map in the contact section is a Google Maps frame and can set cookies of its own. We cover that in detail in the cookie policy.
We would also hand over data if a competent authority required it of us by legal process.
How long we keep it
The request lives as an email. The clinic keeps those emails for as long as it takes to arrange the appointment and follow up, and deletes them afterwards.
If you would rather we deleted yours sooner, ask and we will. Bear in mind that if you go on to become a patient, your medical record does carry retention periods set by Texas law, and those have nothing to do with this website.
What you can ask of us
To tell you what we hold about you, to correct it if it is wrong, or to delete it. Write or call and we will sort it out; there is no charge.
If you are a patient and what you want is access to your medical record, the route is a different one and we explain it at the clinic: those rights come from HIPAA and Texas law, not from this website.
Texas privacy law also gives you the right to have us stop measuring your browsing, and you do not need to write to us for that: the "Cookie preferences" link in the footer switches it off on the spot, and if your browser sends the Global Privacy Control signal we honour it without you having to ask.
Children
This website is not aimed at children under 13 and we do not knowingly collect their data. We do see children at the clinic, of course, but the appointment is always requested by a responsible adult. If you believe a child has sent us data through here, tell us and we will delete it.
Changes
If we change anything material we will update this page and the revision date shown above. Changes take effect when published.
